Documentation
- Students provide documentation to counseling & accessibility services detailing their disability and accommodations recommended by their treating professional
- CAS reviews the documentation and determines the appropriate accommodations
- Students registered are required to inform the instructor each time they plan to utilize their testing accommodations by forwarding a copy of the exam confirmation to the instructor
Accommodation Letters
- Each semester, students who are newly registered or have completed a Semester Accommodation Request Form (SARF) with CAS will give you an accommodation letter
- These letters will provide you with a list of approved accommodations
Confidentiality
- All information provided to CAS by students is confidential and can only be shared
with instructors on a need-to-know basis
Accommodation letters will not list the student’s disability, and instructors should not ask students to disclose. - If you have any questions or concerns about any students registered with the office of counseling and accessibility services, please do not hesitate to contact us
Testing Procedures
- Each student that chooses to use their accommodations must schedule an exam with the
testing center
It is up to each student to decide if they want to use their accommodations for each test - Students are required to schedule exams at least three business days before each test, unless the instructor does not give a 3 day notice. In those cases, we ask the student to schedule the test as soon as they have the opportunity to do so

**It is the faculty’s responsibility to ensure that the accommodations determined and approved by the office of Counseling & Accessibility Services are provided to the student in a timely and responsive manner.**
